Teen Girl Fall Retreat Required Documents
All attendees of the Fall Teen Girl Retreat must have a completed Participation Agreement. Be sure to select "Teen Girls Retreat."
All volunteers over the age of 18 must have completed the California State and AGNCN District requirements for working with minors. These requirements need to be completed and submitted by September 1st. This includes:

1. A completed AGNCN Volunteer Application - only needs to be submitted digitally once.
2. Live Scan Fingerprint Clearance through the AGNCN District - only needs to be done once.
3. A valid California Mandated Reporter Training certificate of completion - needs to be updated every 2 years.

This form is to be used ONLY for necessary special diets due to MEDICAL restrictions. You will be notified prior to the retreat if we are unable to accommodate your request. Thank you!
